    Project Information

    Please provide the following information as it is available or known at the time of application: 

    • Narrative statement including discussion of how the project will support the municipal operation 

    • Map with the project location and subject property identified (e.g. aerial map, tax parcel map). 

    • Project technical specifications if knowne.g. equipment to be installed. 

    • Project timeline. 

    • Project budget. 

    • Any other relevant documents or plans that support this project.

    Permitting, Approvals, Agreements

    • Proof of funding approval/letter of commitment. (Completed bonding or authorizing resolutions will be required prior to contract signing.) 

    • SEQRA Compliance Documentation: Municipalities must document evaluation of project under SEQR (6 NYCRR 617) and compliance as appropriate. If not completed, please document status of review   

    • Identification of federal, state, and/or local approvals or permits if required and timeline for application if needed.
